COHO EMPLOYEE BENEFITS

COBRA requires that group health plans sponsored by employers offer employees and their families the opportunity for a temporary extension of health coverage in certain instances where coverage under the plan would otherwise end.

 

If you were placed on standby in November 2020 your benefits will end 11/30/2020. You will be receiving a COBRA notice by mail that will outline the options available for continuing coverage with our group health plan. If you have moved recently, please let your manager or HR know so your COBRA notice goes to the correct address.  You will have 60 days from 12/1/2020 to elect coverage. Please refer to the FAQ’s – COBRA document or your HR team for any questions.

Health Insurance

Some of the coverage offered includes: annual checkups, prescription medications, access to urgent care facilities and more. The company contributes over 50 percent of the health insurance premiums to support employees and their dependents in this effort.

Dental Insurance

The coverage offered is to support you in your healthy lifestyle by offering annual dental checkups, regular cleanings, x-rays and more. The company contributes over 50 percent of the insurance premiums to support employees in these efforts.

Health Flexible Spending Account (FSA)

This reimbursement account helps you budget some of your income to pay for certain qualified medical expenses not fully covered by your health plan.

Dependent Care Assistance Plan (DCAP)

Designed to help you pay for childcare services for a qualified dependent (as defined by the Internal Revenue Service regulations) when those services make it possible for you or your spouse to work by setting aside pre-tax earnings into a qualified savings account.

Life & AD&D

Life and Accidental Death and Dismemberment insurance is available to full time employees. If you are eligible you will be automatically at one times your annual salary and is 100 percent employer paid.

Long Term Disability

Long term disability insurance is a salary protection coverage should you be injured or become disabled and unable to work for an extended period of time. This coverage is offered and 100 percent paid by the company to eligible employees.

401(k) Retirement Plan

The company offers a retirement plan with a 50 percent contribution match (up to an employee contribution of 6%). Eligibility is defined as 1 year of service, worked at least 1,000 hours during the year and must be at least 21 years of age. If you are deemed eligible you will be notified by payroll and automatically enrolled in the first pay date after your first year anniversary starting at a 3 percent pre-tax contribution rate. You can make changes to your contribution amount or opt out at any time.

Employee Discounts – Available to All employees (Part time & Full Time)

Employees are eligible to receive discounts at Company properties, subject to availability. Written reservation requests must be submitted no less than 2 and no more than 30 days in advance for hotel and restaurant visits. These request forms are available in the ADP Forms Library. The completed, signed, and approved form must be sent to the general manager of the property and a response or confirmation will be given to the employee within 48 hours. A driver’s license or other form of picture identification may be requested before a discount is approved.

 

Employees and their guests must be professional and courteous at all time when visiting any Company property. Any behavior that does not meet this standard will result in exclusion from the discount program and potential disciplinary action, up to and including termination.

 

The Company reserves the right to alter or discontinue any discount program at any time.

 

  • Parking

    • Employees may utilize the park and ride, and 24-hour shuttle service at the Radisson Hotel Portland Airport when traveling to and from the airport. Employees do not have to be a guest of the hotel to use this complimentary benefit. Contact the hotel for availability and reservations.

    • Employees will receive a 50% discount off weekend and evening parking rates at Southpark Garage (914 SW Taylor, Portland, OR), subject to availability. Upon arrival, employees should inform the parking attendant of which Company property they work for.

  • Employees and up to 3 guests may dine at any of the Company’s restaurants (Lakeside Bar & Grill, Southpark Seafood, Hudson’s Bar & Grill) at a 25% discount, excluding alcohol and gratuities.

  • Employees and their immediate family members (spouse, children, grandchildren, siblings, parents or grandparents) are eligible to receive the current friends and family rate being offered by the Company’s hotels (Radisson Hotel Portland Airport, The Heathman Lodge). All rates are subject to applicable taxes and fees.

  • Employees may receive a 50% reduction of the tennis initiation fee or 100% reduction of the fitness initiation fee at West Hills Racquet and Fitness Club or Mountain Park Racquet Club.

Bus Pass Program

To help save on parking and commuting expenses employees are eligible to participate in the Employee Bus Pass Program. This program offers monthly passes to employees at a discounted rate. Please contact your Human Resources department for more information.
